The physicians and clinical team at the Fuqua Center for Late-Life Depression are experts in diagnosis and treatment of depression in older adults. A thorough evaluation is important. Please follow the steps below that outline what to expect on your first visit.
A free self-screening test is available using the Geriatric Depression Scale (GDS) [Long Form] or Geriatric Depression Scale (Short Form). The GDS is a screening tool and is used to determine if further evaluation is needed. Please bring a completed copy with you to your appointment.

If you need assistance in obtaining your medical records, we will be happy to request them from your physician(s) for you. However, in order for us to do so, you will first need to complete one copy of the Consent for Disclosure of Information form for each physician we need to contact. You may complete this form at your first visit or call us at
Plan on your visit taking about an hour and a half. New patient visits include a complete review of health history, medications, and blood work. Please come 30 minutes prior to your scheduled appointment and register on the 4th Floor of the Wesley Woods Health Center.
